Residents of Rotherham over the age of 18 that engage with the service will be able to access a range of lifestyle improvement support tools relating to:
- Weight Management
- Smoking Cessation
- NHS Health Checks (40 to 74 year olds, other eligibility criteria apply)
- Alcohol Screening
- Digital and remote behaviour change support
Self-Referrals
Patients can self-refer via the website and complete a self-referral form.

Professional Referrals
Patients can be referred to the service directly via rotherham.healthwave@nhs.net
- All referral pathways into the service are secure.
- Services offered include smoking cessation, weight management and increase in physical activity.
- Upon receipt of a referral, Rotherham Healthwave staff will carry out a holistic lifestyle assessment of the patient and offer support to bring about changes appropriate to each individual and their circumstances.
